What Happened in the 2022 Data Breach?

The Health Department discovered the breach on November 15, 2022, after detecting suspicious activity, including ransomware. A forensic review later concluded that its system was accessed between October 31 and November 12, 2022, and that a limited number of files were accessed. The settlement defines the covered security incident as occurring between October 31 and November 15, 2022.

In January 2023, the Health Department mailed 115,940 notice letters. They offered a free year of credit monitoring through Equifax and up to $1 million in identity theft insurance.

Opting out: Your request must be in writing, signed and mailed to the administrator. You cannot opt out by phone or email. Do not submit both a claim and an exclusion request. If you do, the claim form will be disregarded.

Objecting: Follow the procedures in the settlement notice, including filing a written objection and sending copies to the required parties. You must also appear at the fairness hearing, in person or through a lawyer. You cannot object if you exclude yourself.

Final Approval Hearing
