What Happened in the American Renal Management Data Breach?

According to the settlement website, an unauthorized actor accessed certain IRC computer systems between February 21, 2024 and March 1, 2024. IRC became aware of suspicious activity on or around February 29, 2024, and began notifying potentially affected people on February 14, 2025.

The information potentially involved included names, addresses, dates of birth, Social Security numbers, driver’s license or state ID numbers, financial account information, taxpayer identification numbers, electronic signatures, health insurance information, and medical information such as billing and claim information, diagnosis or condition, prescription, treatment, medical record numbers, and patient account or identification numbers. Not every person’s information included every category.

The lawsuit alleges negligence, breach of implied contract, unjust enrichment, and violations of consumer protection statutes. American Renal Management denies all allegations and any wrongdoing. The settlement resolves disputed claims. It is not a court finding that the company did anything wrong.

Healthcare data breach settlements like this one are common. For comparison, see our coverage of the General Physician P.C. $2.5M Data Breach Class Action Settlement and the $1.45M OrthopedicsNY Data Breach Settlement. Both claim windows have closed, but the benefit structures are similar.

Who Qualifies for the American Renal Management Settlement?

The settlement class is made up of living people who received a notice from IRC about the data incident, or whom IRC otherwise determined may have had their personal information affected. That includes patients, current employees, and former employees.

  • If you received a notice letter from Innovative Renal Care, you are almost certainly a class member.
  • Eligibility is tied to being notified or identified, not simply to having been an IRC patient or employee at some point.
  • If you think you were affected but never got a letter, call the administrator at (833) 319-0340 or use the contact form on the official website and ask whether you are on the class list.

Excluded: American Renal Management and entities it controls, its officers, directors, legal representatives, successors, subsidiaries, and assigns; judges and court personnel connected with the case; and people who validly opt out.

You do not need to have been a victim of identity theft to claim the basic cash payment.

What Does the American Renal Management Settlement Pay?

The $900,000 fund covers class member benefits, administration costs, court-approved service awards, and court-approved attorneys’ fees and expenses. Class members can choose from three benefits.

1. Pro Rata Cash Payment: Estimated $100

This is a flat payment estimated at about $100. No documentation is required. $100 is only an estimate. “Pro rata” means the amount is calculated after other approved expenses and benefits are paid, so it can be higher or lower depending on the number of valid claims.

American Renal Management $900,000 Data Breach Settlement, $100 Estimated Payment, Up to $5,000, Claim by December 21, 2026

2. Documented Monetary Losses: Up to $5,000

If you lost money because of the breach and can document it, you can claim up to $5,000 per person. Qualifying losses can include:

  • Unreimbursed losses from actual fraud or identity theft
  • Out-of-pocket credit monitoring costs incurred on or after February 14, 2025 through December 21, 2026
  • Unreimbursed bank fees
  • Long-distance telephone charges
  • Postage
  • Mileage for qualifying local travel, at the applicable IRS business mileage rate

The list is not necessarily complete. Other documented, unreimbursed out-of-pocket losses reasonably related to the incident or to efforts to deal with it may also qualify.

What proof is needed. You must provide reasonable third-party documentation, such as bank or credit card statements, phone bills, receipts, or credit monitoring expense records. You cannot claim an expense that another source already reimbursed, including through credit monitoring or identity theft protection IRC previously provided.

3. Two Years of Credit Monitoring

You can claim this on its own or together with a cash benefit. The service includes one-bureau credit monitoring, dark web monitoring, identity theft insurance coverage of up to $1 million, and fully managed identity recovery services. If your claim is approved and the settlement is finally approved, the administrator will send an enrollment code and activation instructions.

Can You Claim Both the $100 Payment and Documented Losses?

Yes. The cash benefits are listed as documented monetary losses and/or the pro rata payment, and credit monitoring can be requested in addition. Review each section of the claim form before you submit.

How the $900,000 Fund Is Distributed

The settlement does not divide $900,000 equally. The fund is first reduced by approved administration costs, service awards, and attorneys’ fees and expenses. The remaining Net Settlement Fund pays, in this order: (1) valid documented loss claims, (2) credit monitoring, and (3) pro rata cash payments. The money left for the pro rata payment is divided by the number of valid claims, which is why the $100 figure is an estimate.

What Are the Lawyers Requesting?

Class counsel will ask the court to approve attorneys’ fees of up to one-third of the fund ($300,000), plus reasonable out-of-pocket expenses. The five representative plaintiffs are asking for service awards of $2,500 each ($12,500 total). The court must approve these amounts before they are paid from the fund.

How to File an American Renal Management Settlement Claim

  1. Find your Class Member ID. It is printed on your notice. The online form asks for it at login.
  2. Go to the online Claim Form or to ircsettlement.com and click “Submit Claim.”
  3. Fill in your contact information. The form then walks you through Documented Monetary Losses, Pro Rata Cash Payment, and Credit Monitoring. Select the benefits you want.
  4. Add payment information so your payment can be delivered.
  5. Sign the attestation and submit.

You can also mail a paper Claim Form, available on the Documents page of the official website. It must be postmarked no later than December 21, 2026. Online claims must be submitted by 11:59 p.m. CT on December 21, 2026.

If you lost your notice and cannot find your Class Member ID, call the administrator at (833) 319-0340 instead of guessing.

Gathering records for a documented loss claim? Do not wait until the last day.

Beware of scams. The official website is the only authorized website for this case. Do not pay anyone to file this claim for you, and only enter your Class Member ID on the official claim form.

What Happens If You Do Not File a Claim?

You will not receive any settlement benefits. If you do nothing, you also stay in the settlement class and give up the right to start or continue a lawsuit against American Renal Management and the other released parties over the claims resolved by the settlement. Simply receiving a notice does not result in a payment.

Your Options in the American Renal Management Settlement

OptionWhat it meansDeadline
File a claimThe only way to receive any benefitDecember 21, 2026
Opt outYou get no benefits but keep the right to sue IRC over the claims this settlement resolves. Requests must be mailed and postmarked by the deadlineNovember 20, 2026

Do nothingNo benefits, and you give up the right to sue IRC over the settled claimsNo deadline

Opting out and objecting are different. Opting out removes you from the settlement. Objecting keeps you in it while you ask the court not to approve it.

How to Opt Out

Send a written Request for Exclusion, postmarked no later than November 20, 2026, following the instructions in the official notice and on the settlement website. It must include your full name, current address, telephone number, email address (if available), the required statement asking to be excluded, and your signature. Group or third-party opt-out requests are not allowed. If you opt out, you receive no settlement benefits.

How to Object

Your objection must follow the requirements in the official notice and FAQ, identify the case, and show that you are a class member. It must be received no later than November 20, 2026. The same deadline applies if you want to give notice that you intend to appear at the final fairness hearing. You may object to the settlement, the attorneys’ fees, the service awards, the release, or other terms.

Because opting out and objecting affect your legal rights, review the Settlement Agreement and consider speaking with a licensed attorney.

American Renal Management Settlement Final Fairness Hearing

The court will hold the final fairness hearing on February 1, 2027, at 9:00 a.m. Central Time at the U.S. District Court, 719 Church St., Courtroom 5C, Nashville, Tennessee 37203. The court will consider whether to approve the settlement, the attorneys’ fees and expenses, and the requested service awards. The date and time can change, so check the official website for updates.

When Will American Renal Management Settlement Payments Be Sent?

There is no confirmed payment date. The court must first grant final approval. If it does, appeals or other challenges could delay the distribution. Benefits will be distributed as soon as possible after final approval and after any appeals are resolved. Do not expect a payment immediately after filing your claim. Filing by December 21, 2026 preserves your chance to receive benefits if the settlement is approved and your claim is valid.
