Do You Qualify to File a Claim?

You may qualify if you provided personal information — including your name, address, Social Security number, or health insurance details — to Anne Arundel Dermatology at any point on or before December 9, 2025.

You may qualify if Anne Arundel Dermatology collected, received, or possessed your personal or health information on or before that same date, even if you never visited the practice yourself.
