Who Qualifies for the Southern Illinois Healthcare Settlement?

You may qualify if you fall into one or more of these groups:

MyChart Portal Users / Online Appointment Schedulers:

  • You may qualify if you signed into the Southern Illinois Healthcare MyChart patient portal or scheduled an appointment through MyChart or website open scheduling from February 21, 2022, to February 28, 2023.
  • You may qualify if you used the same portal or scheduling tool from June 12, 2023, to July 6, 2023.

Health Risk Assessment Completers:

  • You may qualify if you completed a health risk assessment with the defendants from November 2, 2020, to January 31, 2021.
  • You may qualify if you completed a health risk assessment from February 1, 2021, to April 30, 2021.

The full class definition covers all patients of the defendants residing in the United States who fall within any of these categories, totaling approximately 79,215 individuals.

You do NOT qualify if:

  • You are an officer, director, or employee of Southern Illinois Healthcare.
  • You are the judge presiding over the case or their family/staff.
  • You formally opt out of the settlement before June 15, 2026.

How Much Can You Receive?

This settlement offers two separate benefits — and you can take both.

Benefit 1: $17.50 Cash Payment

All class members may claim a one-time $17.50 cash payment. You do not have to provide any proof or explanation to claim this payment. Simply file the claim form before the deadline and select your preferred payment method.

Benefit 2: One Year of CyEx Privacy Shield Pro (Free)

All class members are eligible to enroll in one year of CyEx Privacy Shield Pro, which includes dark web scanning, compromised password scanning, a VPN, a password manager, and other online privacy tools. If anything suspicious happens, you can speak with a fraud resolution agent.

The settlement administrator has already sent CyEx enrollment codes to all class members. Class members do not need to submit a claim form to receive the privacy protection benefit — only to receive the cash payment.

The CyEx Privacy Shield Pro benefit activates after final court approval (August 24, 2026). If you lost your enrollment code, contact the settlement administrator to get a new one before that date.
